Fee For Service Integrated Healthcare Clinician School-Based

POSITION SUMMARY

The Fee-For-Service School-Based Integrated Health Care Clinician provides clinical and case management services for children in a school-based setting. The School-Based Integrated Health Care Clinician will foster positive working relationships with co-workers, clients and external key stakeholders. The School-Based Integrated Health Care Clinician will work in collaboration with teachers, administrators, and parents to provide high-quality mental health services to students in our school district. The School-Based Integrated Health Care Clinician provides leadership, motivation and support to staff, manages the referrals, admission and discharge processes for clients, and ensures the integrity of the program model.
 
*This position is scheduled to start at the beginning of the 2023-2024 school year and is a 12-month position.

EDUCATION AND EXPERIENCE/QUALIFICATIONS

Master’s degree in Marriage and Family Therapy, Social Work, or Counseling, or Doctoral degree in Psychology.

Licensure Requirements for Clinical Positions

Must hold one of the following credentials: LMSW, LCSW, LMFTA, LMFT, LPCA, LPC or licensed or license-eligible Psychologist in the State of Connecticut. Please note that you can apply prior to receiving the above credentials.

ESSENTIAL DUTIES AND RESPONSIBILITIES
 

The School-Based Integrated Healthcare Clinician works as part of a multidisciplinary team to efficiently and effectively meet the behavioral, primary, and dental health needs of the client, including, but not limited to: providing behavioral health consultation to the primary care provider, assisting client in meeting medical treatment compliance, and referring client to other Wheeler services and/or other community services as needed.
 

• Conducts program specific screening and evaluation, and formulates diagnosis in accordance with the Diagnostic Statistical Manual (DSM). Develops and documents, clear clinical formulations and treatment plans that are individualized and measurable, developed in collaboration with the client/family and others as relevant, using behavioral health screening as directed, and are inclusive of behavioral health, primary care, and dental goals as indicated. 
• Demonstrates knowledge of psychopathology and primary modalities of psychotherapy, including individual, group and family. Correctly triages clinical emergencies, including suicide and danger to others. Uses best practices and evidenced based practice models for psychotherapy where appropriate. 
• Makes effective use of topic oriented, focused, time-limited group therapy. Provides or refers to group when required. 
• Demonstrates knowledge of the use of psychotropic medications. 
• Completes clinical documentation in timely manner.
• Maintains and facilitates a minimum of 1 group during the school year, and utilizes evidence based practices (EBPs) to engage clients in model specific treatment targeted to address trauma.
• Demonstrates knowledge of internal and external levels of care, referral processes and resources. Makes clinically appropriate referrals for identified service needs in a timely manner. 
• Communicates in a positive, timely, effective manner with client, family, and all internal and external providers a minimum of once monthly. Attends any required meetings and multidisciplinary treatment team planning meeting, providing a maximum of 1 hour per week of consultation to their designated school.
• Meets direct service productivity goals as defined by program. Demonstrates knowledge of program/department/agency goals and target outcomes and adheres to practice standards and to data collection and reporting protocols.
• Documents all client related information in compliance with agency policy.
• Attends and is prepared for scheduled supervision. Seeks additional supervision or consultation as needed and follows through with supervisory directives.
• Provides clinical leadership by implementing, promoting, and sustaining evidenced based and best practice models of prevention, intervention, and treatment.
• Provides guidance and crisis support to staff.
• Works with manager or designee to develop, monitor, and analyze clinical and program performance and outcomes and develops and monitors quality improvement program goals. 
• Interfaces and collaborates with and other community providers to provide positive system development for clients and families, to promote Wheeler’s reputation as a quality service provider. 
• Reports child abuse/neglect as mandated, adheres to confidentiality guidelines and ensures that the program meets all necessary licensing, regulatory, and accreditation standards, as related to the program, staff, records, facility, and care of youth.
• Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.
• Regulatory and Corporate Compliance Responsibilities
• Understands and adheres to Wheeler Clinic’s compliance standards as they appear in the clinic’s Corporate Compliance Plan, Code of Conduct, and Conflict of Interest Policy.
• Completes all training in a timely manner as required by state and federal regulations (e.g., Compliance, Fire Safety, HIPAA HITECH, Infection Control, Sexual Harassment, etc) and specific to the department and position.
• Keeps abreast of all pertinent federal, state and clinic regulations, laws, and policies as they presently exist and as they change or are modified.
• Ensures that the staff are trained and evaluated on their knowledge of and adherence to compliance policies and procedures specific to their jobs.
• Performs other related duties as assigned.

1. Education and Experience / Qualifications: Master’s degree in Marriage and Family Therapy, Counseling, Social Work or related license eligible field or a doctoral degree in Psychology is required.

• Demonstrated ability to assess clinical emergencies, including suicide and danger to others and reaches disposition according to established protocols.
• Demonstrated ability to complete clinical documentation/electronic medical record entry in a timely and professional manner as dictated by program standards.
• Demonstrated knowledge and skill in utilizing culturally appropriate treatment techniques sensitive to client cultural traditions, linguistic backgrounds, values and beliefs.
• Demonstrated ability to collaborate and function as a part of an integrated care team demonstrating knowledge and application of the principles and best practices in care integration.
• Demonstrated ability to maintain therapeutic boundaries.
• Demonstrated ability to identify the clinical need for a psychiatric or medical evaluation and demonstrate skills and knowledge to promote medication adherence through education, behavioral intervention, family supports, and collaboration with prescribing nurses and physicians.
• Clinician demonstrates consistent implementation of health promoting interventions.
• Evidence of coordination with primary care assessment and care delivery.
• Clinician demonstrates knowledge of basic medical terminology, and basic baseline health indicators and potential signs and symptoms of comorbid medical-behavioral health conditions relevant to population served.

4. Physical Demands: This position requires the ability to sit, drive, use of hands to manipulate or feel, and talk or hear, and to stand, walk, the ability to reach with hands and arms, stoop, kneel, crouch, crawl, or lift up to 25 pounds. The vision requirements for this position include having clear vision at less than 20 inches, clear distance vision greater than 20 feet, peripheral vision, and depth perception.

• Some of the specific job duties that require the physical demands include, using a computer, reading, driving to meetings and different locations, and stocking supplies.

5. Work Environment: This position risks exposure to bloodborne pathogens and hazardous waste. This position will have a moderate noise level.
