Job Summary

Under the supervision of the Team Lead, the Activities Coordinator will provide on-site assistance to tenants to help them maintain their housing. The Activities Coordinator assumes responsibility for coordinating programs and services to help promote the tenants of quality of life, including providing direct assistance and modeling for activities of daily living (ADLs).

 

Our on-site Activities Coordinator provides tenants with information, makes referrals to community resources and service, and brings resources on site. The Activities Coordinator will assist tenants in developing plans and goals, and teach independent living skills (e.g., cooking, cleaning, budgeting, organization, job skills, time management, communication/coping/problem-solving skills, etc.). The Activities Coordinator will be responsible for facilitating groups, helping tenants take care of basic needs like access to food and transportation, and providing opportunities in the building to enhance the sense of community and belonging. The Activities Coordinator will collaborate with tenants to develop housing inspection plans.

 

Essential Job Duties & Responsibilities

  • Assists with the development of a therapeutic setting.
  • Develops and maintains collaborative relationships with community partners.
  • Develops a sense of fellowship and community using organization and facilitation skills. This may involve the creation of social/community-oriented programming, opportunities, tenant associations, community-based newsletters, welcoming committees, orientation packages, support groups, etc.
  • Engages all tenants in the building(s) to identify areas of need and makes referrals to internal and external resources when necessary.
  • Identifies and distributes information to link tenants to community resources.
  • Creates and fosters a network of local social services agencies and facilitate tenants’ access to programs.
  • Assumes responsibility for coordinating services for tenants on an individual- and goup-level basis.
  • Provides quality customer service to all tenants including, but not limited to, answering questions, addressing concerns, and assisting with basic needs.
  • Refers tenants to local service providers aligned with the residents’ personal goals, interests, and needs; follows up to ensure appropriate service delivery.
  • Raises awareness among appropriate providers of the needs of tenants of the community and promotes partnerships that help tenants gain access to needed services.
  • Maintains appropriate documentation in tenants’ files.
  • Sponsors educational events that include subjects relating to health care, agency support, life skills, and referral sources.
  • Implements on-site or mobile health services and screenings.
  • Organizes and promotes tenants' involvement in on- and off-site programs.
  • Assists residents in acquiring and utilizing desired community services such as housekeeping, meals, transportation, personal services, financial assistance, daycare counseling, and other services.
  • Transports clients to appointments as needed.
  • Ensures all tenants are treated with respect and dignity, reports all complaints made by tenants and/or families to the appropriate supervisors, reports all allegations of abuse to the appropriate authority, and any other corporate compliance items immediately.

Other Job Duties & Responsibilities

  • Participates in various standing, functional, and ad hoc committee groups.
  • Assumes supervisory functions in the temporary absence of other supervisors and managers.

Job Qualifications

High school diploma or GED with one or more years of experience in a social service delivery with mental health and/or substance use and/or unhousedpopulation. Demonstrated working knowledge of supportive services and other resources in the area served by the project. Demonstrated ability to advocate, organize, problem-solve, and provide results for the residents served.

 

Physical Demands/Work Environment

This job operates in a professional office environment and, as necessary, transport clients to appointments. While performing the duties of this job, the employee is regularly required to speak and hear. The employee is frequently required to sit for extended periods, stand, walk, use hands and fingers, and reach with hands and arms. Travel and work outside of normal hours may be required for meetings, presentations, training, and other events.
