JOB SUMMARY

The Centers seeks one Substance Use Counselor for our ACT Team.

This Substance Use Disorder Counselor will work closely with our multidisciplinary team to provide substance use services to persons in the community. The counselor will be the lead SUD service provider on the team
and will provide ACT clients with integrated dual diagnosis disorder treatment. The SUD Counselor is responsible for providing confidential counseling, education, and support to individuals and families experiencing substance use disorders, and for promoting health lifestyles and healthy choices to individuals, families, and the community. Provide a range of prevention, assessment, and intervention services. Provide training and consultation to team members on problems, assessments, and recommendations for clients. Use IDDT treatment and Motivational Interviewing techniques.

Job Qualifications

  • Requires at least one of the following licenses through Ohio board of Counselor, Social Worker and Marriage & Family Therapist (CSWMFT) or Ohio Chemical Dependency Professionals Board:
    • LCDC-II, LSW, LCDC-III, LPC, LICDC, LISW, LPCC
  • Must possess a high degree of initiative, orientation toward process improvement, and demonstrated leadership qualities.
  • Must have
  • Must possess computer proficiency and proficiency working in an EMR.
  • Must understand and embrace a systems approach to service delivery, as well as, organizational mission, program philosophy, goals, and organizational values.
 

Essential Job Duties & Responsibilities

  • Provide individual, group counseling, crisis intervention and case management services.
  • Provide counseling to pregnant women about the effects of drugs and alcohol on the fetus, as requested.
  • Will utilize evidence based approaches to care, including motivational interviewing, trauma informed care and strength based therapy.
  • Work collaboratively with behavioral health, physical health and pharmacy staff to ensure integrated care is available to all clients.
  • Develop treatment plans with client and team that are complete, individualized and based on needs from assessment and those identified in the course of treatment.
  • Document accurately and completely all care provided using appropriate records and within allotted time frame.
  • Actively monitor outcomes and make real time adjustment to ensure program meets targets.
  • Develop and facilitate communication and education plan to help increase addictions awareness among BH staff.

Physical Demands/Work Environment

This job operates in a professional office environment and the community. This role routinely uses standard office equipment. While performing the duties of this job, the employee is regularly required to speak and hear. The employee is frequently required to sit for extended periods of time, stand, walk, use hands and fingers, and reach with hands and arms. It requires the ability to lift files, open filing cabinets and bend or stand on a stool as necessary.
