Position Summary:

SYHealth’s Center for Research & Health Promotion augments the health center’s clinical care with health promotion, research and care coordination programs for patients and community members. Under the direct supervision of the specified Program Supervisor, the Program Coordinator will help coordinate various aspects of programs, as well as provide guidance and oversight of activities. The Program Coordinator will be responsible for the effective functioning of program activities, the efficient use of program resources, and for timely reaching program goals and objectives.

 

For the purposes of this position, the Program Coordinator will coordinate the Elevate Youth Program. The Program Coordinator will be responsible for all aspects of the peer group including planning and facilitating meetings, coordinating with other project support staff and other activities related to the project. The Program Coordinator is responsible for assisting and facilitating the group development, program design and implementation and ensuring program deliverables and outcomes are on track to be met. In addition, the Program Coordinator will help coordinate outreach in schools and community agencies, marketing, & working with the assigned team to conduct project activities.

 

Essential Functions of the Job:

  • Assists in the development, coordination, implementation, and evaluation of program activities.
  • Recruits and oversees a select group of youth aged 14-18 from local schools and community agencies.
  • Maintains files, tracks attendance, and provides coaching and mentorship.
  • Coordinates with the Project staff to conduct project activities and outcomes (i.e., recruitment, developing group norms, needs assessment, and project design and implementation).
  • Implements the program activities into teen clinic and school operations, working with staff, to minimize the burden on ongoing clinical services, and assuring the effective functioning of the project.
  • Assists the Program Supervisor to coordinate activities with internal staff and group members at outreach events.
  • May be asked to participate in field activities, with the purpose of creating awareness of SYH facilities and services including health education and patient self -management programs.
  • Helps implement and coordinate the administration of evaluation activities such as pre and post-tests or surveys to gauge knowledge, satisfaction, and other skills.
  • Assists in the coordination of trainings and other activities to benefit the members.
  • Establishes and maintains linkages with both in-house resources and community partners, assuring their effective use to achieve program goals and objectives.
  • Completes operations and training goals and objectives.
  • Completes reports required by funding entities in a timely fashion.
  • Participates in program meetings with collaborators at different sites thru ought San Diego, representing SYH and the Center for Research and Health Promotion.
  • Ensures that staff adheres to SYH policies and procedures.

 

Additional Duties and Responsibilities:

  • Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.
  • Enhances professional growth and development through participation in educational programs, current literature, in-service meetings and workshops.
  • Maintains program specific records as required; submits reports, including activity, contact and mileage reports in accordance with agency policies and procedures.
  • Attends training programs as authorized or directed.
  • Attends meetings and/or training sessions as required and participates on committees as directed.
  • Performs other related duties as assigned or requested.

 

Job Requirements


Education and Experience Required:

High School Diploma or GED equivalent and minimum of 2 years of experience providing health education in group and/or individual settings. OR Any combination of training and experience which would provide the required knowledge, skills, and abilities. Examples of qualifying experience include:

  • Graduation from an accredited four-year college or university with a bachelor’s degree in a health-related
  • field and 1 year of health education experience.
  • 2 years in a position comparable to Health Educator counselor.
  • Other combinations of education and experience may be considered.

 

Additional Experience Required:

  • Experience in coordinating programs in the public health field; or minimum 1 year experience in community health; health education.
  • Experience implementing evidence-based curriculum and trainings. 

 

Certifications Required:

  • Valid CA Driver’s license and vehicle insurance
  • Regular & reliable access to personal vehicle

Technical Knowledge and Skills Required to Perform the Job:

  • Experienced in program implementation & documentation.
  • Ability to handle multiple activities simultaneously.
  • Capability to establish and maintain good interpersonal relationships.
  • Ability to work independently and meet deadlines.
  • Must maintain confidentiality at all levels. 

 

Equipment Used:

  • General office equipment to include Computer, fax, phones, printer, scanners, projectors, Microsoft Office (Word, Excel, and Outlook), etc.

 

Working Conditions and Physical Requirements:

  • This position is intended to be a hybrid role, where the incumbent will be expected to work “On-Site” two days per week.
  • Prolonged sitting position. Frequent walking and standing at outreach events. Be able to carry a minimum of 20 pounds.
  • May be required to work evening and weekend hours. Traveling within San Diego is expected in this role.

 

Universal Requirements:
