Job Summary
To act as an advocate and assist patients/clients in gaining access to resources. Identifies and understands the social and emotional factors underlying the patient's/client's illness. Provide supportive counseling, social work case management, and clinical treatment for patients/clients and their families/support systems.  

Essential Functions
A. Must behave ethically and professionally.
B. Must be capable of working with diverse populations.
C. Must have emotional resilience and stability as the work often involves dealing with crises.
D. Must have knowledge in caring for patients in different developmental stages throughout the life cycle.
E. Provide individual treatment and/or counseling, and social casework for patients and their families.
F. Serve as a member of the treatment team, working to diagnose problems, formulate treatment plans, and evaluate progress.
G. Prepares socio-psychiatric case history information, psychosocial assessment, and social work treatment plans for use in screening and diagnosis.
H. Maintains cooperative working relationships with community agencies, schools, long-term care facilities, and other medical facilities.
I. Participates in education, meetings, and serves on committees.
J. Maintain records and prepare reports and correspondence. 
K. Complete all necessary assessments including, but not limited to psychosocial, financial, and assessment of current support systems and assist with appropriate discharge planning.
L. Assist patients/families in completing necessary economic and healthcare-related applications.
M. Complete all required documentation to comply with Medicare Conditions of Participation and PMC policies.
N. Maintain patient confidentiality.
O. Maintain current knowledge of rules and regulations of government programs by partnering with state and local agencies including but not limited Department of Health and Human Services, Public Health Department, etc.
P. Responsible for making regulatory reports with other agencies including, but not limited to:

  1. Law enforcement and
  2. Child Protective Services or Adult Protective Services. 

Q. Facilitate group support sessions as needed.
R. Regular attendance at the assigned work location is an essential function of the job.    

Education/Experience
A. Certified Masters Social Worker degree preferred; and 
B. Licensed Mental Health Practitioner (LMHP) or Licensed Independent Mental Health Practitioner (LIMHP) preferred. 
C. One year of experience in a healthcare setting is preferred.
