Concordia-IRMC VNA is seeking a full-time Registered Nurse Clinical Supervisor to join their hospice team. The VNA has been a well-established Home Health/Hospice provider in the Indiana and surrounding counties for more than 50 years. The Registered Nurse Clinical Supervisor oversees the management and coordination of the day to day operations involving the Hospice Home and Community. Provides appropriate end of life nursing care to all patients and families based on individual plan of care, and in accordance with established regulations and administrative policies.
 

QUALIFICATIONS

  • Registered Nurse with a current valid Pennsylvania license.
  • Two (2) years of progressive experience in a hospice setting recommended.
  • Interpersonal, communication and organizational skills.
  • Proficient and knowledgeable of an electronic health record.
  • Knowledge base of agency policies, federal and state regulations and third-party payers
  • Reliable, independent means of transportation to make home visits. Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state

RESPONSIBILITIES

  • Reviews clinical documentation for compliance and completeness in a timely manner.
  • Reviews Plan of Care (485) for accuracy and completeness.
  • Review HIS metrics
  • Oversees and offers supervision to members of their team to help assure that quality of care is provided.
  • Holds regular meetings to provide education to team members and facilitate interdisciplinary communication.
  • Acts as liaison between patient/family, physician and all other healthcare providers.
  • Provides in-home nursing visits on an as-needed basis.
  • Participates in administrative on-call rotation.
  • Collaborates with the patients and the clinical staff when needed when abnormal reading or trending is identified.
